All the information on this page or linked on this page is required reading for all MathPath students and their families.
Pre-Program Briefing. Click here to open the 2020 version. This document contains a wealth of information to prepare students and families for MathPath. It is an expectation that students and families will be familiar with all the information in this document upon arrival at MathPath.
Required submissions.

Headshot. Instructions for submission can be found here. As stated in the instructions, the headshot is used for internal MathPath purposes only.

Waiver and Agreement Form. Agreement to pay MathPath for all charged tuition/medical/damage/other fees assessed to your family by MathPath, agreement that our protocols as outlined in the Pre-Program Briefing reflect due diligence, and restrictions on legal claims.

Behavioral Standards Agreement. This form clearly sets out our behavioral expectations for students at MathPath, including potential consequences of failing to adhere to them. MathPath families may consider using this form as an opportunity to discuss proper behavior in a group educational setting.

Personal Electronics Agreement. Acknowledgement of understanding and agreement to abide by our electronics policy, including the prohibition of all high-tech electronics except a cell phone.

Photo and Quotation Permission Form. Release for MathPath to make public use of photographs and statements made by all MathPath students as long as all students are not identified without prior permission.

Health Form. Provides essential health, medication, insurance, diet, behavior, and emergency information about each MathPath student. Requires signatures of both parent/guardian and pediatrician/family doctor. Requires current physical (within 12 months of the start of MathPath).
Before completing this form, MathPath families must have or obtain insurance that fully covers urgent and emergency care at medical facilities in the area. ("Urgent care" and "emergency care" are defined terms in healthcare policies, which distinguish the care from "routine care", which can wait until the student gets home.) Coverage should be effective from June 28 until August 1 and must also cover any hospitalization extending beyond August 1.
Disclosure of any current or previous conditions (including diet and behavior) is essential for MathPath staff to properly support each MathPath student. International Students: Foreign government health insurance does not cover the holder in the US. Temporary US insurance must be obtained.
Recommended Submissions.

Recreational Activities Waiver Form. Required for any student who may want to participate in a weekend or evening activity that would require a signed waiver (such as tubing, rock-climbing, trampoline parks, etc). This form allows MathPath staff to authorize participation in such events on behalf of parents/guardians. If this form is not submitted, the MathPath student will not be able to participate in such activities – MathPath staff will not contact families on an individual basis for participation in any activity. Parents/guardians may restrict activities by specifically listing them on the Waiver Form.
Optional Submissions.

Student Handover Permission Form. Designation of persons other than parents/guardians who may take custody of a student during or at the end of MathPath. Agreement to abide by the Off-Campus Visit procedure outlined in the Pre-Program Briefing.
Parents/guardians may designate up to three other individuals to take their MathPath student off-campus (including to pick them up at the end of the program). Without this form, MathPath will not release students to anyone except the parents/guardians. Do not submit this form unless someone other than a parent/guardian may pick up the student.
Submitting Completed Forms. Please precisely follow the instructions below regarding form submission.
- All submissions are due on or before May 31.
- Submit all forms at the same time. (Wait until all have been completed.)
- Submit forms in the order they are listed above.
- Put any scans of insurance cards after page 1 of the health form.
- Include any extra documentation provided by a doctor after page 4 of the health form.
- Forms may be submitted as hardcopy (via postal mail), or as scans via email. Instructions for both listed below (when possible, MathPath prefers to receive electronic versions of the forms).

- The scan must be dark, clear, legible/readable, and sufficiently large.
- Do not send a series of cell phone photos. Use a scanner or a scanner-app on a smartphone. Verify that any scans created by a smartphone app are legible.
- The attachement should be a single pdf file.
- Name the pdf file <Surname>_<GivenName>_Forms.pdf. Form submissions for a student named Emma Lee should have a file named Lee_Emma_Forms.pdf. Please use underscores, not hyphens.
- Submissions should be less than 3Mb in size. To achieve a smaller file size, use a "text" or "line art" setting on a scanner instead of "photo"; or adjust the dpi setting (240 or fewer).
- Submit to both Program Director April Verser and the MathPath Admissions Assistant. Their addresses (click once for both) are

- Use the subject line "Completed MathPath Forms". Include the student's name if desired.
Acknowledgment of Receipt. MathPath will acknowledge receipt of all documentation, usually within two business days. If you do not received acknowledgment within a reasonable time, please inquire by email with the person to whom you sent it.
